Summary Acute aortic dissection is an extremely serious condition with a high mortality risk. Symptoms may mimic other emergencies such as myocardial ischemia, often leading to misdiagnosis. A combination between coronary hypoperfusion and relatively rare, but when present it could be fatal. Early diagnosis correct surgical approach are essential success in those patients the rate requires improvement of protection. Development acute cardiac ischemia on top mandatory global precondition for severe postcardiotomy syndrome, only excellent protection can lead successful intervention.
